1. The coach’s name
The coach’s name should be inputted in order to secure accountability.
2. The participant/client’s name
The participant’s name should be written down in this part.
3. Beginning date of the training
The starting date of the training should be indicated here.
4. Ending date of the training
The ending date of the training should be written down here.
6. Skill assessment
In this part, the coach should indicate the existing skills of the participant in order to assess the right training plan for them.
7. The coach’s observations
In this section, you could include a review of concerns/questions that were the focus of the coaching, the description of the coaching process, the necessary recommendations of the coach, and the results of the efforts to implement based on the recommendation of the coach.
In the summary, the coach’s job is to bring the whole training process together. The results of the training should be included here– such as the participant’s improvements and so forth.
