Steps in Making a Survey Form

When making a survey, the first thing that people usually do is think of the questions that are going to be in it. This is a very wrong way of doing a Survey Form. Making one is a process and certain steps need to be followed so you can make a survey form that is effective. Outlined below are the steps you need to follow when making a survey form.

  • Make goals and objectives. This is the first step in making a survey questionnaire. You have to define your goals for making the survey. Types of Survey Questions

In any survey forms, it is important to choose a type of survey questions and stick to it for the entirety of the form. Of course, you can have other types also included in there depending on what the question requires but make sure to stick to only one type for the majority of the document. Listed below are the different types of survey questions and how your respondents could answer them.

  • Rating Scale Questions – This is the type of survey question where respondents are presented a rating scale to rank how they feel about a certain product or service. When using this type, make sure you are consistent with your rating scale. For example, when you have a rating scale of 1 to 10 for the first question, do not use a 1 to 5 scale for the next.
  • Multiple Choice Questions – In this type, you are giving the answers to your respondents. They just have to choose which one applies to them. Multiple choice questions have the option of having multiple answer selections.
  • Rank Order Questions – This type of survey questions also gives the respondents answers to the questions. They then rank the choices based on their personal preferences.
  • Dichotomous Questions – This is the type of survey question where respondents are given two options. In a survey form, they often appear as questions that are answerable by yes or no, true or false, agree or disagree.
  • Open-Ended Questions – These are also called comment or essay box questions because respondents are asked to provide their own answer to the questions. They are not predefined answers, so they are free to write whatever they want.

Tips in Making Survey Forms

  • Keep it short. It is a fact that shorter forms have a better response rates. Long forms to fill out will definitely scare your respondents. Make sure it is short but also covers the objectives that you have set for that survey.

  • Provide balanced options. In questions where you provide options to respondents, make sure to have equal numbers of negative and positive options. A neutral option should also be provided so you get accurate results.
