Surveys are done so that usable data could be drawn out of a certain population. When you want to know what your target population is thinking, you take a sample out of that number and ask them relevant questions about the things you want to know. Surveys can be done online, on the phone, or face to face. In a survey, the sample is asked a set of predetermined questions that accurately assess their thoughts and opinions.

Survey forms are very effective tools in picking out the brains of your population sample. Our Sample Survey Forms does exactly just that. You can print out our sample survey forms and use them for your own.

What Is a Survey Form?

A survey form is a document that is used to evaluate the thoughts and opinions of a sample from a given population. It contains a set of predetermined questions that are expected to accurately assess what the customers think about a certain thing, place, etc. The survey questions in these forms need to cover all aspects of the topic being researched about.

Surveys forms are particularly very useful business tools. Through them, businesses can collect data and interpret that data into useful results that they can use in making decisions about their business. When done right, these businesses will be able to minimize risks and improve the products and services that they provide their customers. This would in turn result in greater customer satisfaction.

Why Companies Do Surveys

When talking about measuring public opinion, there is no better way of doing it than getting it personally from them. Surveys are the best option when you want to know what they are thinking. They have been proven to be valuable instruments in almost any fields of research. Nowadays, almost anyone can make a survey about almost anything.

Businesses, however, are the most common users of this tool. Companies remain alive because of the people that continue to avail of their products and service and so it is very important to listen to what they are saying. Aside from the customer side of things, a survey reveals much more about the business such as industry trends, product awareness, competition, and so on. There are many reasons why businesses would want to conduct a survey research and some of them are outlined below.

  • To see customer satisfaction levels. Customers are the lifeline of any business and keeping them satisfied is the main goal of companies. Doing a survey would tell companies how satisfied their customers are of their product and/or service.
  • To see expansion opportunities. It is a goal for many businesses to grow. Doing a survey is a great way to see whether they still have customers in other markets. Surveys are great indicators that tell whether expanding to other markets or market segments is viable. Check out our sample feasibility reports when you think of expanding your business.
  • To prove or disprove a hypothesis about their customers, competitors, etc. Businesses may have noticed a trend of, say, the buying behavior of their customers. One way of proving or disproving this is through conducting a survey. Surveys are great tools in testing the validity of the hypothesis. When you have established industry trends, you hold very valuable data.
  • To see problems in the business and fix it. When customers see a loophole in the business, they often would want to communicate that to the owner but don’t know how. Surveys are the best way for businesses to see what needs improvement and deal with it.
  • To get feedback on new and existing products. Businesses would want to know what their customers think of their products. When a new product is launched, a feedback would also be needed to see what customers think about it.

Steps in Making a Survey Form

When making a survey, the first thing that people usually do is think of the questions that are going to be in it. This is a very wrong way of doing a Survey Form. Making one is a process and certain steps need to be followed so you can make a survey form that is effective. Outlined below are the steps you need to follow when making a survey form.

  • Make goals and objectives. This is the first step in making a survey questionnaire. You have to define your goals for making the survey. After that, make a list of objectives that you want to achieve. This is a very important step in the process as this provides the foundation with which you are going to formulate your questions.
  • Formulate the survey questions. This is now the part where you make a list of the questions to be included in your survey questionnaire. Make sure that the questions are made with the objectives in mind so that they are able to address them. Make as many questions as you can so you have plenty of options to choose from.
  • Pick out the best questions and make them into only one “type” of survey questions. For this stage, you will need to select questions from the pool of questions that you have formulated from the previous step. After choosing which questions will make it to the survey form, think about how they will be structured in the form. There are many “types” of survey questions and they will be discussed in the next section.
  • Test the survey form. After you have finalized everything in the questionnaire, you may be thinking it is ready to face your sample population. You need to hold up for a while because a series of testing needs to be done first to make sure it accomplishes the objectives you have set. First, test it out to people in the organization. It is also a good time to proofread the questions. The second testing is done using a small portion of the population sample.
  • Send it out to your sample population. Once all the above steps are done, the survey form is now ready for distribution to your respondents. By this time, you should have already determined your target population and the number of sample from that population that you need to survey.

Types of Survey Questions

In any survey forms, it is important to choose a type of survey questions and stick to it for the entirety of the form. Of course, you can have other types also included in there depending on what the question requires but make sure to stick to only one type for the majority of the document. Listed below are the different types of survey questions and how your respondents could answer them.

  • Rating Scale Questions – This is the type of survey question where respondents are presented a rating scale to rank how they feel about a certain product or service. When using this type, make sure you are consistent with your rating scale. For example, when you have a rating scale of 1 to 10 for the first question, do not use a 1 to 5 scale for the next.
  • Multiple Choice Questions – In this type, you are giving the answers to your respondents. They just have to choose which one applies to them. Multiple choice questions have the option of having multiple answer selections.
  • Rank Order Questions – This type of survey questions also gives the respondents answers to the questions. They then rank the choices based on their personal preferences.
  • Dichotomous Questions – This is the type of survey question where respondents are given two options. In a survey form, they often appear as questions that are answerable by yes or no, true or false, agree or disagree.
  • Open-Ended Questions – These are also called comment or essay box questions because respondents are asked to provide their own answer to the questions. They are not predefined answers, so they are free to write whatever they want.

Tips in Making Survey Forms

Survey forms are essential tools every business needs to have in their arsenal. Making one is not a task that is to be taken lightly. Here are some tips you can use when you are making one.

  • Keep it short. It is a fact that shorter forms have a better response rates. Long forms to fill out will definitely scare your respondents. Make sure it is short but also covers the objectives that you have set for that survey.
  • Use close-ended questions as much as possible. First of all, close-ended questions are much easier to answer for respondents as opposed to questions that require them to think of their own answer. Second, they are much easier to analyze and interpret.
  • Present questions in logical order. When answering survey questions, respondents should see a progression. Start out with a broad question and then narrow it down in the following questions.
  • Provide balanced options. In questions where you provide options to respondents, make sure to have equal numbers of negative and positive options. A neutral option should also be provided so you get accurate results.

Surveys are essential tools in all industries and they are very useful in businesses. Survey forms are very much like Evaluation Forms in that they are tools used in seeing how people think. Our sample survey forms are printable so you can use them. They are very easy to use and are tailor-made with business objectives in mind.
