- Make goals and objectives. This is the first step in making a survey questionnaire. You have to define your goals for making the survey. After that, make a list of objectives that you want to achieve. This is a very important step in the process as this provides the foundation with which you are going to formulate your questions.
- Formulate the survey questions. This is now the part where you make a list of the questions to be included in your survey questionnaire. - Pick out the best questions and make them into only one “type” of survey questions. Types of Survey Questions
In any survey forms, it is important to choose a type of survey questions and stick to it for the entirety of the form. - Multiple Choice Questions – In this type, you are giving the answers to your respondents. They just have to choose which one applies to them. Multiple choice questions have the option of having multiple answer selections.
- Rank Order Questions – This type of survey questions also gives the respondents answers to the questions. They then rank the choices based on their personal preferences.
- Dichotomous Questions – This is the type of survey question where respondents are given two options. In a survey form, they often appear as questions that are answerable by yes or no, true or false, agree or disagree.
- Open-Ended Questions – These are also called comment or essay box questions because respondents are asked to provide their own answer to the questions. They are not predefined answers, so they are free to write whatever they want.
