Inclusions of an Employee Disciplinary Action Form

For proper recording of the disciplinary action needed to take place, an employee disciplinary action form must include the following information:

  • the name of the employee who is subjected for disciplinary action;
  • the job position of the employee and the area, division, or department of the company where he is assigned at;
  • the name of the employee’s supervisor or manager to assure that the head of the department knows the action that will be given to their employee;
  • the level of warnings that are give to the employee regarding his or her warnings;
  • the date when the disciplinary action has been made;
  • the incident that is the reason for the disciplinary action;
  • the date and time the incident happened;
  • a brief description of the incident and the people who are involved;
  • the action plan needed to be applied to provide corrective measures to an employee; and
  • the agreement between the employer and the employee about the next action to be done should a specific action or problem be reported again.

On Suspension and Termination

The action plan to be applied in a specific incident may depend on the rule book information and guidelines of the company regarding an action that needs corrective measures. An employee may be subjected for suspension or termination depending on the action that he or she has done and on how it has affected the company in specific areas.

  • If subject for suspension, an employee disciplinary action plan must enumerate the dates the employee is required to not render work, the days where the employee will not be paid,  the other items that the employee is required to do in line with his or her suspension, and the date in which regular work functions may already be done by an employee during his or her return for work.
  • If subject for termination, the company should write all the guidelines that they have followed to come up with the decision. They should also include the date when the termination will be effective and the processes that are needed to be done by the terminated employee before leaving the company.
