AVID or Advancement Via Individual Determination is a tutorial program for middle and high school students intended to help them be ready for college. It offers elective courses that develop diligence and other necessary skills for student course taking. It also helps students understand their core courses better, thus, they can be successful in their future classes. One of the things that you have to comply to avail of the service is a tutorial request form:

A tutorial request form is a document that guides the items and activities in a tutoring process. It usually has some of the following details:

• the name of the student enrolled or in need of the service;
• the date, time, and semester the tutorial is rendered;
• the areas or items that must be covered and discussed in the service; and
• the methods or techniques that can be employed in the tutorial.
