A laboratory incident report is a tool that records any event that may or may not have caused injuries to a person or damage to any laboratory equipment.   • Name of the lab supervisor if applicable

III. Type of Incident

Identify the type of incident such as injury, near miss, property damage, or theft.

IV. Type of Injury and Damages

If the person(s) involved did get hurt in the process identify what type of injury and the which area in the body has been injured. If there are any damages on the property, describe this in detail.

V. Details of the Incident

Describe how the incident occurred such as the nature of the incident, if an injury occurred, indicate circumstances and who was involved. Indicate if any substances or objects were involved. What was done to protect individuals or clean up the substance? Also indicate if emergency personnel were contacted and if there the person(s) were transported to hospital.

VII. Conclusion or Recommendation

Define what were the primary & contributing causes of the incident and what could be done to prevent this from reoccurring.
