Basic Information

With this basic information, it will be easier for you to organize and discern your clients’ feedback records.

Feedback Questions

As mentioned on the previous posts about feedback forms, there are different types of feedback form styles. However in this case, it depends on the coach’s preference. They come in survey, essay, or multiple choice.

This is the multiple choice and it comes with fixated answer choices, and this includes the following:

  • Satisfied
  • Very Satisfied
  • Excellent

Or it could be in a form of a “yes” or “no” choices.
