Physical assessments or physical exams are commonly dubbed as checkups and is a process of physically inspecting a patient’s body for any signs of disease, ailments, or medical conditions that might pose a medical risk in the future.

A physical assessment is recommended for everyone regardless of age, sex, color, race, or level of activity and is strongly recommended for people over the age of 50 in order for clinicians and health practitioners to detect earlier any signs or symptoms of disease and to devise a treatment plan to control or eliminate it. Our Sample Assessments can be utilized to help document the findings or results of a physical assessment.

When a clinician conducts an assessment, they typically interview the patient first. With the use of an assessment form or Nursing Assessment Forms, clinicians and health-care practitioners take down details pertaining to a patient’s medical record such as his medical history, current and previous doctors and their contact information, maintenance medicines, history of surgery or operations, previous diagnosis, etc.

There are generally varying types of physical assessments that differ according to their uses. Below is a list of the common types of physical assessments:

Routine Physicals

Routine physicals are primarily conducted for medical screening purposes and are done on asymptomatic patients—patients who do not harbor any signs or symptoms of illness.

Routine physicals are usually conducted by a physician, a primary care provider, or a licensed nurse and typically follows the HEENT physical examination which goes through a physical assessment of the head, ears, eyes, nose, and throat.

Preemployment Physicals

Preemployment physical assessments are usually conducted to screen applicants and make sure that they are fit enough to work. Employers typically require newly hired employees or applicants to go through this process to make sure that employees will have fewer absences due to sickness or injuries and will have slimmer chances of occupational diseases.

Comprehensive Physicals

A comprehensive physical assessment is a more in-depth type of physical examination because it typically includes medical tests that are comprehensively and rigorously done such as laboratory tests, chest X-rays, CAT cans, EKGs, mammograms, etc.

Insurance Exams

Insurance physical exams are typically conducted by health or life insurance personnel when purchasing a health or a life insurance policy.

Sports Physicals

Sports physical assessments or pre-participation physical examination is a type of assessment that helps to determine if it is safe for an individual to join or take part in a particular sport. Most sports doctors believe that an individual’s medical history is the most important part of a sports assessment and should therefore be provided clearly and accurately.

Assessment forms play a vital role in the proper documentation and organization of the results of a physical exam. By using assessment forms such as our Self Assessment Forms and Training Assessment Forms, it is easier for you to conduct physical examinations and to record them in a clear and organized manner for future references.
