  • Request a fraud alert and freeze your credit. Any institution that pulls up your credit report will be notified that your account may be compromised. They’ll take extra steps in identifying the person trying to open the account. Putting a freeze on your account provides additional protection as it prevents the three credit bureaus from sharing your report to anyone who requests it. You can get an annual credit report form from the three credit bureaus.
  • Review your credit reports. This is how you’ll find out if any new accounts were mysteriously created using your personal and financial details. The official website to request for a copy of your credit report is at AnnualCreditReport.com.
  • Check unauthorized charges on your credit and debit cards. If you notice any charges that you do not have knowledge of or don’t remembering making, immediately report it to your financial institution. Request your account to be locked or closed.
  • Open new financial accounts. What’s the point of using compromised accounts? Open new financial accounts and make sure that your new information won’t be leaked to the identity thieves. This way you can prevent further damage and becoming a repeat victim.

How to Get an Affidavit for an Identity Theft

Follow the steps below to help you get an affidavit that is used for identity theft.

Step #1: Get an Identity Theft Affidavit Form

The Federal Trade Commission has an Identity Theft Affidavit Form that is recognized and accepted by many business institutions. In some cases, the form can be used in place of a police report. You may also contact the business or institution to ask the procedure that they follow and the Identity Theft Affidavit form they use.

Step #2: Fill out the required information

You’ll be required to put in sensitive personal information, like current address, Social Security Number, and driver’s license number on the form. Additional supporting documents may also be needed.

Step #3: Provide your statement of declaration

After completing the rest of the form, you’ll need a declaration statement detailing the issues surrounding the identity theft. Declaration statements should only state facts because false statements are punishable by the law.
