This document starts with indicating the investment amount at the top plus the date of issue. Next, the document should always have the names of the parties involved. For the company getting the debt, include its name and the representative present during the transaction and for the debt lender, write down his or her name, or the companies name, then, below that, write the name of the agent present to administer the agreement.
